If your daughter is interested in playing a Spring 2026 sport, please complete the registration form that was emailed to all registered student families.

DIAA Sports Physicals Update
 

There has been an update to the Ursuline Academy and DIAA Physical Form process. Please review the information below carefully, as requirements now vary by grade level and whether your child participates in athletics.

LOWER SCHOOL: All Students in Early Childhood - Grade 5

• Beginning this upcoming school year, all students will be required to submit a new physical each school year. The Delaware Student Health Physical form is linked here. Physicals must be completed within one calendar year of the upcoming school year and renewed annually before the previous physical expires.
• This includes all current and new Early Childhood-5th graders.
• Please note that if you are new to Ursuline, you can complete this step once you receive access to Magnus Health in UA Connect later this month.

1. Upload a scanned copy (or clear photo) of your child's physical in the Magnus Health Account in UA Connect
2. All forms must be completed and uploaded upon expiration of the previous year

MIDDLE & UPPER SCHOOL: All Students in Grades 6-12

Step 1: Physical Requirement for Ursuline

• Every student must have a new 2026-27 DIAA Physical Form on file to attend school. The new physical form is linked here. This will be the only form accepted for the 2026-27 school year.
• The required forms are the DIAA Health History Form, Page 3, and Physical Form, Page 4 (even if not playing a sport, these are the forms that are being used)
• These forms must be dated after April 1, 2026, for the 2026-27 school year
• If this does not match your annual physical date and your child is an athlete, the doctor can sign off on the athlete's clearance to participate with a “clearance date.”
• Upload Pages 3 & 4 to Magnus Health in UA Connect
• Upload a scanned copy of the completed physical form to your child's Magnus Health Account by August 1, 2026
• Please note that if you are new to Ursuline, you can complete this step once you receive access to Magnus Health in UA Connect later this month.

Step 2: Physical Requirement for DIAA (only complete this step if your child will participate in a sport during the 2026-27 school year)

Create a Rank One Account: 

DIAA requires all parents to create an account on Rank One and upload the required documents for their children to be eligible to participate in interscholastic athletics.

MULTIPLE CHILDREN AT DIFFERENT SCHOOLS
If you have multiple students who attend different schools or districts, you can claim all of them to your parent account. For example, if you have a son at Salesianum, but a daughter at Padua or Ursuline, you can still claim them under a single sign in, but you will need to make sure you switch to the other school before trying to claim them.  

1.) You only have to create a parent account ONE time. 
2.) Go to www.rankone.com, click the "Parents Click Here" button.
3.) Select "Go to Forms"
4.) Select your State
5.) Select one of the school districts that one of your students attends. 
6.) You can either create an account or login & claim your student. 
7.) To claim another student at a different district, follow the previous steps again and select the other school district your other student(s) attends.

Complete these steps in Rank One by August 3, 2026:

1. Page 2 of the 2026-27 DIAA Physical Form (Eligibility Form) – digitally complete this page to proceed with the other action items
2. Page 5 of the 2026-27 DIAA Physical Form (Medical Card) - upload the completed form as a jpeg or pdf
3. Pre-Participation Consent Form - digitally consent to the Sudden Cardiac Arrest Awareness, Heat Policy Awareness, the Concussion Awareness, and Consent to Treat policies.
4. Emergency Contact Card - upload the completed form as a jpeg or pdf
5. Double-check that each form is submitted and shows as "Complete" in green. As a reminder, these forms must be dated after April 1, 2026, for the 2026-27 school year
